How To Access Our Services

Thank you for your self-referral and enquiry.

Please review our admission criteria before submitting your details.

• Live with a life limiting illness and prognosis
• Requiring symptom management
• Live within Cittamani Hospice’s catchment area (https://www.cittamanihospice.com.au/area-we-cover/ )
• Has a full-time live-in carer
• Agreement to and awareness of the self-referral to Cittamani Hospice
• Agreement to the primary goals of Cittamani Hospice – to control symptoms, offer psychosocial support, maintain quality of life, and respect the client’s/family’s end of life wishes

Once we have received your self-referral, someone from our Clinical Team will make contact with you to discuss any additional information we may need.

Once a referral is accepted, our Clinical Team will work collaboratively with your GP and any other health professionals involved in your care to ensure that your care is coordinated effectively.
